Well, how can I say this? You are right from one point of view and not right from another. I have been deeply involved in all aspects of this particular dynamic for many years now. The private insurers are certainly influenced by many things in their negotiating stance with a particular facility but both the providers (hospitals and doctors) and the payors each have some reasonable idea of the number of patients (and inferentially the amount of money involved) that will result from a provider participating at a particular level.

It is the facilities choice to be included or not included with every payor except Medicare where they have no choice at all and must accept what the government pays - full stop! This is one of the reasons why the "bill master" (the regular retail published prices) is always so high. It is not like they actually expect anyone to pay it but in a kind of 'kabuki theater' way it is the place where the negotiations often begin. There are so many dynamics (variables) in this equation it is hard to empirically prove anything.

I think we both fully understand how the government's heavy handed participation in the markets tends to distort and amplify the underlying dynamics. The single biggest detriment to a true market place in the medical economy is the government. Every other participant is rational and this would lead to a rational market without the big dumb hand of the government pushing its way into every nook and cranny of the otherwise reasonable decisions made by the other participants.

This mornings Boston Herald.

Note: ;Tim Cahill is running against Gov. Patrick as an independent.  The only way Patrick can get re-elected in my opinion is if he splits the democrat vote.  We'll see.  The Mass. Health care system is not well and Tim Cahill being the State Treasurer should know.

Tim Cahill slams Barack Obama, Dems on health care

State Treasurer Tim Cahill, taking swipes at both Gov. Deval Patrick and President Obama, boosted his bipartisan chops yesterday, telling Herald columnist Howie Carr on WRKO, “I voted for John McCain, believe it or not.”

Cahill, saying he was barred from the 2008 Democratic National Convention because he wouldn’t endorse either Obama or Hillary Clinton, said, “My own party basically voted me out.”

“I was afraid of what we had already been getting in Massachusetts, and at that point in 2008, I was aware that it wasn’t working,” he said. Separately yesterday, Cahill accused Obama of “propping up” the Bay State’s health plan with federal aid in order to help push the Democrats’ plan through Congress.

“The real problem is that this . . . sucking sound of money has been going into this health-care reform,” Cahill said. “And I would argue that it’s being propped up so that the federal government and the Obama administration can drive it through.”

Gov. Deval Patrick argues the state’s universal health care program has added 1 percent to the budget, but Cahill said the real impact is buffered by federal dollars.

Meanwhile, Republican Charles Baker’s campaign said Patrick “has consistently failed to address rising health-care costs in Massachusetts.” Baker, the former Harvard Pilgrim CEO, advocated for years for greater transparency on the part of medical service providers.

Cahill called on congressional Democrats yesterday to go “back to the drawing board,” saying he fears they will “bankrupt” the country.

Patrick’s campaign yesterday used Cahill’s health-care smackdown in its latest fund-raising pitch, e-mailing supporters that Cahill “is advocating policies that could put that access, and their health, in jeopardy.” Patrick, whose administration held a hearing on health-care costs yesterday, said exorbitant premium increases and medical service costs need to be curbed through legislation he has proposed.

Listening to Pelosi discuss the plus side of the HCB. The new bill will cut 500 bil. of fraud and waste from Medicare. Why aren't the politicians going after those cuts regardless of the Health Care Bill? Quote "I love numbers, they are so precise" while at the same time they have changed the numbers every few days and they have no proof of the numbers the cite. How is that precise? And how about the 72 hours given to read the bill and be prepared to make an intelligent vote on Sunday?

To quote Mrs. Doubtfire "Brace yourself Effie".

It's only swamp land until they dredge up some reef material and build a good solid base of sludge land to set the pilings in. Screw the fish and aquatic wildlife, They have the whole ocean to swim in. (This is all in my most sarcastic voice possible)
